Roles & Responsibilities

Evaluate residents to develop individualized, goal-driven therapy plans
Deliver skilled therapeutic interventions to improve mobility, strength, and daily function
Collaborate with nursing and interdisciplinary teams to coordinate care and optimize outcomes
Document progress clearly and timely in the patient chart to support continuity of care
Educate residents and families on safe mobility, fall prevention, and home exercise programs
Monitor outcomes and adjust treatment plans to meet changing needs and regulatory standards

Qualifications

Active, unrestricted Physical Therapist license (Montana) or ability to obtain state licensure promptly
Current Basic Life Support (BLS) certification (or willingness to obtain before start)
Experience in skilled nursing, rehabilitation, or post-acute care preferred
Strong clinical assessment and manual therapy skills with sound clinical reasoning
Excellent communication skills and a collaborative attitude toward interdisciplinary care
Ability to pass facility onboarding and background checks for a rapid start

Education

Doctor of Physical Therapy (DPT) preferred; Master's in Physical Therapy or equivalent considered

Knowledge & Skills

Skilled in assessment of mobility, balance, transfers, and functional independence
Proficient with therapeutic exercise, gait training, and assistive device recommendations
Knowledgeable about documentation standards for skilled nursing and Medicare/Medicaid requirements
Comfortable with patient and family education and discharge planning
Able to prioritize caseloads and manage time efficiently in a busy rehab environment
