Job summary
Provide one hour of individual counseling and 2 hours of group counseling weekly, and family counseling as scheduled and/or assigned, documenting residents' and families' response to intervention.
Responsibilities
- Complete assessments and screenings, resident's initial individual treatment plan to include setting goals and objectives, transition planning, and safety plan within 30 days of intake.
- Complete the therapy/counseling, substance abuse/chemical dependency, permanency, screening tool, and discharge information components of the resident's initial individual master treatment plan MTP reviews
