Experience & Requirements:
- Graduated from a Licensed Practical Nursing program
- Experience with drug abuse is mandatory
- Currently holds a Practical Nurse license in the state of employment
- Must have an active CPR certification
- Stays up-to-date on specific state laws and regulations governing practice
- Successful completion of initial and annual clinical competencies assigned to the role. A background check, security clearance, and drug screening are mandatory for this position.

Responsibilities:

Available Positions:
Pay: $30.50 to $33.00 per hour
Shifts Available: Day Shift, Night Shift with Rotating Weekends
Hour: 3*12 (7:00 AM to 7:00 PM and 07:00 PM to 07:00 AM)

Shift Differential:

Can be up to $3.00
Sign-on Bonus: $6,500 Corrections Below is a list of your duties as a Licensed Practical Nurse (LPN) at YesCare:

- Contributes to evaluating patients' health status
- Assists in developing and adjusting patient care plans
- Carries out delegated tasks within their educational background and practice scope
- Delivers basic care to multiple clients within the healthcare program
- Ensures safe and effective nursing care and takes accountability for the care provided
- Conducts patient education following established teaching plans
- Uses problem-solving to guide decision-making
- Monitors scheduling both offsite and onsite with a collaborative approach
- Supervises the consult process in line with policy and contract
- Takes part in Quality Improvement initiatives
- Participates in Patient Care Committee Meetings
- Oversees CARES procedures as required
- Ensures follow-up care for patients onsite and offsite, including telemedicine appointments

Will collaborate with Regional Medical Director, site medical providers, Regional Nursing Director, and other site nursing leaders.
