- Assists and advocates on behalf of the individual; upholds respectful and efficient communication with individuals and all relevant parties.
- Provides emotional support by developing an age-appropriate relationship with individuals.
- Assists individuals in experiencing all phases of the Pathways Program, both on-site and off-site including transportation, event participation, learning, re-learning programs and socialization opportunities.
- Aids individuals in developing and maintaining daily living skills, encouraging independence, personal hygiene, behavior (i.e., appropriate responses/actions with others including other individuals and the public), proper dietary and mealtime habits, and safety in and out of the community.
- Performs direct support and supervision, including but not limited to:
- Obtains and maintains certification as a Medication Passer.
- Attends appropriate in-services and continuing education.
- Maintains strict confidentiality of individual and personnel information.
- Collects and records data regarding individual well-being, progress, health, unusual incidents, personal goals and other information.
- Understands and complies with job requirements as outlined in Agency policies/procedures, e.g., mandated reporting, personnel policies, individual rights, etc.
- Gives input and information regarding individuals' individual plans, health concerns and behavioral plans.
- Completes documentation daily and communicates with staff, individuals, family, medical professionals, and other personnel in reference to individuals.
- Demonstrates a strong commitment to the mission and values of the agency.
- Adheres to agency's attendance standards.
- Performs other related duties as assigned.
- Eighteen (18) years of age.
- Must have a valid Ohio Driver's License with a record suitable for insurability purposes
- Driver's license must have less than the maximum number of points per agency policy
- Must have reliable transportation and two years minimum driving experience
- Must provide proof of a High School Diploma or equivalent
- Criminal background that is compliant with OAC 5123:2-2-02 Employment of persons providing services to individuals with DD
- Must be compliance with agency drug free workplace program
- Must have the ability to communicate and interact with individuals, families, and staff maintaining the highest level of respect and service
- Must have the ability to deal with pressure, problems, and difficult situations, while maintaining perspective and emotional stability
- Ability to complete all required training.
- Cooperative, patient, and caring nature.
- Communicate and interact with individuals, families, and staff, maintaining the highest level of respect and service.
- Ability to participate in all in-service trainings and a minimum of 8 hours annual continuing education.
- Ability to participate in orientation and on the job training.
- Dependable, demonstrated adequate writing skills, good moral character and able to work in stressful situations in a calm, therapeutic manner.
- Experience working with individuals with Developmental Disabilities is preferred.
- May be exposed to infectious illnesses.
- May be exposed to aggressive individuals.
- Requires physical exertion including but not limited to walking, bending, reaching, kneeling, squatting, pushing, pulling, and lifting 50 lbs. or more. These may be in excess of light duty work.
- Requires corrected hearing and vision to normal range.
