Join our dynamic team as a part-time Nurse Practitioner (NP)or Physician Assistant (PA), where you will engage with 2-5 patients daily inthe comfort of their homes. As a practitioner and educator, you will leverageyour expertise as an NP or PA to provide transitional and chronic care,mitigating hospital readmissions and emergency room visits.

Responsibilities
Conduct thorough examinations of patients and review theirmedical records
Summarize medical records from acute settings
Order and interpret diagnostic tests such as blood tests andX-rays
Diagnose health conditions and propose effective treatments
Prescribe medications as needed
Maintain accurate records and schedules
Inform patients about their health conditions and prescribedmedications
Educate patients and their families on diseases, epidemics,and prevention
Oversee patients' overall health and promote good healthpractices
Collaborate with other healthcare providers involved inpatient care
Skills
Proven experience as a Nurse Practitioner or PhysicianAssistant in various healthcare settings
Expertise in primary care, acute care, and chronic carepractices
