Provide care and support to patients in their homes during evenings, nights, holidays and weekends.

Responsibilities
- Maintain up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.
- Provide patient/family visits based on assessed need, hospice policy and procedure, and established schedule.
- Collaborate with attending physician to confirm existing orders change orders acquire new orders as needed appropriate.
