- Address phone inquiries, resolve issues, facilitate care coordination, and identify requirements
- Perform data entry, create charts, generate reports, and undertake other healthcare tasks
- Efficiently manage calendars and schedules on a daily basis
- Attend to referrals and charting one weekend per month
- Support clinicians with daily challenges related to patient needs
- Undertake projects as necessary
- Track care coordination and case management
- Communicate with both medical and non-medical staff
- Prepare for patient visits by organizing charts, notes, and medical equipment
