Job Summary
We are seeking a dedicated and compassionate Case Manager to join our Adult Foster Care (AFC) team.In this role you will assess the needs of individuals served match them with appropriate caregivers and ensure ongoing placements meet program standards.
You'll provide advocacy education and ongoing support to both individuals and caregivers Key Responsibilities Assess individual needs coordinate appropriate caregiver placements Conduct regular home visits Ensure all program requirements compliance standards are met Qualifications Bachelor's degree in Social Work A license from the Massachusetts Board of Registration in Social Work 2+ years of related experience working with elderly or disabled adults OR Bachelor's degree in a related field 2+ years of clinical experience working with elderly or disabled adultsJob description
