The Care Managers main purpose is to ensure their client caseload is provided with the necessary services they need in a coordinated effective manner Responsible for the caring compassionate delivery of case management advocacy other supportive services to ensure appropriate coordination of services for clients Qualifications Required HS diploma or GED plus 4 years in Behavioral Health combined Behavioral Health education experience meet Behavioral Health Technician Qualifications One year case management experience strongly preferred Ability create plans summaries based on client needs Certification first aid CPR Crisis De escalation training will offered do not possess upon hire Must at least 21 years age older Possess valid drivers license acceptable driving record reliable transportation Prior experience behavioral health setting strongly preferred Demonstrated organizational abilities maintain communication coordination connections clinical team participants Willingness work evenings needed Desire capacity maintain time-management organization working fast-paced hands-on work environment Must obtain maintain fingerprint clearance card pass pre employment drug background check Have Tuberculosis screening test provide evidence freedom infectious tuberculosis prior providing services additonal TB testing screening questionnaires performed annually CPR First Aid Certification training provided candidate does not possess active certification Possess physical mental cognitive ability perform specific clinical tasks required position posing direct threat safety security patients Mental Emotional Ability prioritize workload Sound judgment decision-making ability Maintain calm non-defensive supportive attitude difficult interactions Demonstrate excellent verbal communication skills Analyze variables situations Resolve problems handle conflict make effective decisions under pressure Think feet public spotlight Physical Long periods sitting ability bend lift carry pounds Operate phone Fax copier comput Able operate Agency vehicle Bilingual English Spanish Experience Integrated Healthcare Behavioral Health Primary Care environment Advanced knowledge cultural diversity sensitivity
