- Assesses each patient, identifying treatment issues, needs, resources and care goals.
- Completes or insures completion of field specific assessment for each patient.
- Coordinates with patients and an interdisciplinary team to develop, implement, monitor, and update treatment plans
- Conducts individual, family, and group processes according to program guidelines.
- Provides counselling to patients and families with a diagnosis of cancer.
- Schedules outpatient follow-up psychotherapy appointments for patients and/or family members with treating therapist.
- Documents case management and therapeutic services according to department standards.
- Acts as a resource for patients, families, and staff regarding various treatment matters.
- Under direction of supervisor/manager and the Cancer Committee, assists in the evaluation of needs and implementation of processes for effective program improvement and development.
- Promotes teamwork with Oncologists, Cancer Navigator and personnel of other departments.
- Participates in patient and community psychoeducational programs related to cancer.
- Acts as the Psycho -Social Coordinator for the MWH Regional Cancer Committee, ensures data collection of all distress screening data for the Commission on Cancer.
- Performs other duties as assigned.
- Bachelor's degree in Social Work or related behavioral health field required.
- LCSW, LPC, CSAC or license in clinical psychology required. License must be valid in Virginia.
- Previous inpatient/outpatient mental health/substance abuse experience required.
- CPR (AHA Healthcare Provider) required.
- Experience in assessment and evidence-based/best practices of psychological treatment of mental health disorders and psychosocial problems relevant to a cancer and other chronic or life limiting illnesses diagnosis or serious illness in an inpatient or outpatient clinical setting, individually and with groups.
