Job DescriptionJob Title: Care Coordinator
Requires in person client visits.
Responsibilities:
1. Care Coordinator may assume responsibility for conducting the initial Intake including:
a. Visiting the client at home, shelter or other location.
b. Complete an intake form.
c. Identifying issues that may lead to crises and making appropriate referrals.
d. Providing information and referral for ineligible applicants.
e. Assisting the client in understanding and signing the application form.
2. Coordinating client counts
3. Participating in the care conference and care plan development.
4. Set up, arrange and monitor client services and document accordingly.
5. Make appropriate referrals to community resources
6. Regular and monthly monitoring client's well-being and/or contacting family as assigned.
7. Verifying services and case recording.
Minimum Requirements:
Bachelors degree required in social work, psychology, sociology or related field. Two years of direct service experience providing in home visits conducting long
term community care services.
Physical Demands:
· Driving daily to visit clients in homes, shelters and other areas and locations.
· Lifting capability of 5-10 pounds (depending on what is taken in the field) laptop, paperwork, etc.
Other Requirements
Valid California driver's license and proof of current liability insurance required. This position requires local travel for home assessments.
