Join Our Mission at Lambs Farm At Lambs Farm we've spent over six decades helping people with developmental disabilities lead productive happy lives through meaningful work opportunities in our farmyard restaurants stores and community programs We're expanding our home-based services to help more families access the support they need creating an exciting opportunity for someone passionate about participant empowerment to shape how we deliver individualized services in community settings As our Self-Directed Assistant for Home-Based Services you'll work directly with participants and families to help them manage their funding and direct their own services Based in Libertyville with 50% travel to participant homes throughout the area you'll provide the guidance and support that enables families to make informed choices about the services that matter most to them This is your chance to help participants and families take control of their services build meaningful relationships within a supportive culture.
