    Schedule Available: 8:00 AM to 4:30 PM, Monday through FridayMomentum is currently hiring an Adult Day Program Caregiver Coordinator to join our team The Adult Day Program Driver is responsible for providing transportation for people supported at Momentum locations in the San Fernando Valley to and from the Day Program in Thousand Oaks.

    In this dual hybrid role, in addition to providing transportation, this staff member will also work at the Day Program in Thousand Oaks.

    Ensures safe operation of the residential transportation vehicles in accordance with Momentum's Operational ManualSafely operates Momentum vehicles on surface streets, freeways, highways, and interstates in compliance with DMV and Highway Patrol protocolsOperates vehicle lift for people supported in a safe manner and ensures wheelchairs are secured properly.

    Safety policy requires a 4-point securement for all mobility devices.

    Assists people attending the program with program activities and community-based activitiesAssists people attending the program with communication, feeding, mobility, toileting, hygiene, and other activities of daily livingEngages in program activities including computer class, art class, sensory activities, and gardeningFollows company policies regarding the safe and proper lift and transfer of persons supportedPerforms other duties as assigned to meet department and/or business needsADP Caregiver Coordinator Education and Experience:A high school diploma, GED, or equivalent; required A minimum of two (2) years of related work experience; required Experience working with individuals with developmental disabilities; preferred Skills and Knowledge: Must be able to read, write, comprehend, and communicate effectively in EnglishMust have three (3) years driving experienceAbility to maintain confidentialityRequirements: Valid and current proof of legal right to drive in California; requiredMust have three (3) years driving experience; required Current DMV driving record printout; requiredSuccessfully pass all post-offer, pre-employment screening requirements, including criminal background check, drug screening, physical exam, and provide proof of eligibility to work in the United StatesAbility to run, push, pull, bend, squat, kneel, walk, stand and sit for long periods of time, and lift up to fifty (50) pounds*Onetime Sign on Bonus will be paid in two payments.

    First payment will be made after 90 days of active employment and second payment will be made after 6 months to active employees in good standing.
