- Operations: Serve as a resource for field staff pertaining to clinical issues and/or concerns. Facilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to manager.
- Patient Care: Work in the field with staff and patients as needed.
- Customer Service: Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.
- Minimize Patient Risk: Focus on Outcome Monitors as assigned.
- Policies: Review clinical documentation and maintain current knowledge of all Medicare/ Federal/ Insurance / Company and Hospital guidelines, policies and protocol.
- Teamwork: Assist with monthly Case Manager, Nurse and Therapy meetings. Participate in all on-call requirements, and patient staffing as needed.
- Compassionate communicator with a positive attitude.
- Patience is a virtue when working with patients, families, physicians, and coworkers.
- Attention to detail is critical, as is being observant and following directions.
- Critical thinker and the ability to problem solve clinical needs.
- Current LPN license in the state of employment
- Minimum of Two (2) years of experience as an LPN in an acute care setting
- Minimum one-year home health experience is preferred
- Valid driver's license and auto insurance in your name as a driver
- Capable of all physical demands

Schedule: Monday - Friday, 8:15AM - 5:00PM. On call per business needs.
