    JOB SUMMARY Works under the supervision of the Manager of Cardiac and Pulmonary Rehab and cooperatively with medical staff.#

    Assesses, plans, implements, and evaluates teaching, exercise, and discharge planning needs of the cardiovascular patient in Phase I, II, or III Cardiac Rehabilitation and Pulmonary Rehabilitation.

    # Observes the standards of the policies and operating instructions of Memorial Healthcare.#

    Strives for superior performance by consistently providing a product of service to leadership and staff that is recognized as ultimately contributing to the patient and family experience.

    PRIMARY JOB RESPONSIBILITIES 1.#Assesses the physical, psychological, and social dimensions and the educational needs of the patient upon referral and completes a written assessment. 2.#Plans for the teaching and learning needs of the cardiac patients in Phase I, II, and III along with pulmonary patients. 3.#Implements a plan of care including proper activity levels, education, and discharge planning needs. 4.#Evaluates the plan of care, recognizes changes in status of the patient, and provides appropriate intervention and documentation. 5.#Monitors and evaluates patients during exercise modalities in Phases I, II and III with emergency intervention as indicated.# 6.#Acts as a resource person regarding care of cardiac and pulmonary patients to staff, community. 7.#Participates in in-service and/or continuing education to maintain and improve understand-ing of the present and future trends in healthcare, and participates in unit and hospital wide committees to identify needs and assist in problem solving. 8.#Communicates identified unit problems following appropriate chain of command (Manager of Cardiac and Pulmonary Rehab, Corporate Director of Operations), and actively participates in the problem-solving process. 9.#Demonstrates job related aspects of patient safety to assure a safe environment, safe and effective use of equipment and technology, and decreased risk of potential adverse patient occurrences. 10.#Demonstrates knowledge of and supports hospital mission, vision, value statements, standards, policies and procedures, operating instructions, confidentiality statements, corporate compliance plan, customer service standards, and the code of ethical behavior. 11.#Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of patients served on his/her assigned unit.# Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient#s status and interpret the appropriate information needed to identify each patient#s requirements relative to his/her age-specific needs, and to provide the care needed as described in the unit#s and division#s policies and procedures. 12.#Performs other job-related duties as assigned. #

    Communicate receptively and expressively in English, both written and verbal, with diverse populations including physicians, patients, families, community agencies, employees, and volunteers.

    B.#Visually assess patient#s condition and to auscultate lung and heart sounds, blood pressures, etc.# C.#See and interpret cardiac monitor. D.#Accurately hear communication by phone or in person. E.#Maintain self control. 7.#INTERPERSONAL SKILLS# A.#

    Interact effectively with patients and their families, public, physician and other Memorial Healthcare staff to provide appropriate and complete patient care.

    • EDUCATION
    A. Graduate from an accredited school of Science or Nursing.

    B. BS in Science or nursing or related field preferred.

    C. ACLS certification required.
