    Description

    POSITION SUMMARY: Provides alcohol and drug screening, assessment, referral, prevention education and outreach case management to pregnant/postpartum (up to the end of the month in which 60 days falls) women.


    QUALIFICATIONS: Bachelor's Degree in Behavioral Science field (i.e., Community Health Education, Social Work). Must have appropriate certification in the State of Kentucky. Must receive Certified Prevention Professional (CPP) credential within 2 years of employment.


    SKILL SETS: Possesses superior organizational, oral, and written communication skills. Must demonstrate a high level of familiarity with Microsoft Office specifically MS-Excel and exceptional customer service skills.


    PHYSICAL AND ENVIORMENTAL DEMANDS: Frequent communication verbally by phone, email and face to face with individuals and groups in the community. Manual dexterity required for written communications. Must be able to stand for long periods of time in order to provide training. Visual acuity, hearing and speaking in order to interact with clients and the community. Frequent travel to eight counties and must be able to work inconsistent hours on weekends and nights as needed. Must be able to lift heavy objects.


    JOB SPECIFIC FUNCTIONS:

    1. Provides alcohol and drug screening to pregnant/postpartum women, as evidenced by the Level 1 Screening Tool in the clients' chart.
    2. Provides educational sessions to pregnant/postpartum women, regarding the affects of alcohol, tobacco, and other drugs on an unborn fetus, as evidenced by the data entry report.
    3. Provides prevention services to Medicaid recipients in accordance with 907 KAR 3:220 as evidenced by the Quality Control Checks performed on a quarterly basis.
    4. Engages high-risk pregnant women in outreach case management services, complete Baseline screening and Service Plan as evidenced by the clients' chart.
    5. Submits regular monthly reports to prevention coordinator and substance abuse clinical director based on receiving reports from the MIS manager.
    6. Attends training as set forth by the state.
    7. Ensures that medical record documentation and relevant correspondences are completed as required, as evidenced in the clients' charts and reported in the quarterly Quality Control Checks.
    8. Collaborates and maintains positive relationships with key providers and ensures community coalition maintains active role in the development and implementation of pregnant/ postpartum women intervention services, as evidenced by the KIDS NOW semi-annual regional meeting and data entry report.
    9. Market the program to agencies and community partners and provide speaking engagements.
    10. Collects documentation and statistics to determine program outcomes as reported in the data system.
    11. Provides consultation, technical assistance and education services to other agencies regarding KIDS NOW Program as evidenced by the data entry reports.
    12. Provides written documentation of services delivered that is entered into the data base by the 10th of each month to the.
    13. Performs other duties as assigned.

    MANDATORY REQUIREMENTS:

    1. Adheres to Communicare, Inc. policies and procedures.
    2. Attends mandatory in-services and other training/educational programs as required.
