    Community Health Specialist


    Under the general direction of the Region 9/10 Family and Community Health Services (FCHS) Manager, and direct supervision by the FCHS Nursing Supervisor, Performs complex (journey-level) public health work.

    Work involves planning and developing both public health education program activities and vaccination events. Assists with population-based outreach activities, clerical, and clinical duties which may include vaccinating clients.

    The service area includes the Big Spring area and surrounding counties in addition to other offices and regional area satellite sites.

    Responsibilities includes assisting Team Lead and field office nurse in the provision of health services; help program staff and the public in areas of public health, community health, and prevention services; client screening and eligibility determination; assisting with preparing and submitting end of the month reports; ordering office, clinical, and vaccine supplies.

    Maintains records for the Field Office clinic and satellite sites.

    Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.

    Essential Job Functions:
    (35%) COMMUNITY ACTIVITIES
    Plan, Coordinate and evaluate COVID Vaccination or testing events. Coordinates, plans, and monitors public health education activities; and develops goals and objectives for public health education programs.

    Provides and educates communities, stakeholders and others on COVID, COVID Vaccines, Maternal Health, Perinatal Infant Health, Child Health, and Tobacco prevention information through presentations, events, trainings, and community networking.

    Obtains Train-the-Trainer certifications for Mental Health First Aid, Suicide Prevention, Sex Trafficking and other population-based focus subject matters. Collaborates with FCHS team to plan, develop, and coordinate program outreach activities and services. Promotes program(s) to community and professional groups to encourage and/or improve interest in the program(s) and improve community outcomes.

    Participates in local, regional and statewide initiatives to improve public health infrastructure and capacity to respond to emerging public health issues.

    (35%) CLINICAL DUTIES:

    Provides client screening and eligibility determination services. Assist nurse in exam rooms.

    Escort patients to exam rooms, interviews patients, measure vital signs, including weight, blood pressure, pulse, temperature, vaccinations and document all information in patient's chart.

    Files lab results in appropriate client record. Assists with administration of Directly Observed Therapy medications to Tuberculosis client(s) as needed under the guidance of the RN. Assist with screening clients for vaccine history and prepares vaccine information documents according to DSHS immunizations schedule. May collect, process, and prepare laboratory specimens for shipment and maintenance of log.

    CLERICAL DUTIES:
    Assist in the day-to-day office functions of public health sub-offices, field offices, and clinics. Prepares reports on services rendered by using clinical data software and maintains client reminder tracking system. Assists in obtaining, verifying and/or transfer client medical information according to HIPAA guidelines. Compiles new client records. Maintain, file, and secure confidentiality of all client records. Assists with ordering/receiving clinical and vaccine supplies. Assist in monitoring the temperature of vaccine storage unit(s) regularly to assure that appropriate conditions are maintained. Assist with office supply and ordering.

    (5%) OTHER RELATED DUTIES AS ASSIGNED

    Other related duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations activation.

    Such participating may require an alternate shift pattern assignment and/or location.

    Safe operation of either a state-owned vehicle or personal vehicle is required to conduct state business and an acceptable driving record must be maintained in accordance with DSHS Policy OS-, Fleet Operations and Management & Procedures and standards developed by the Texas Department of Public Safety.

    Complete all trainings related to job duties on a timely manner.

    • Ability to maintain efficient and organized systems to ensure efficient patient flow.
    • Ability to be trained in performing phlebotomy and administering tuberculosis skin tests.
    • Ability to interpret public health policies, procedures, regulations, and laws.
    • Ability to communicate effectively both written and orally.
    • Ability to use standard computer software including word processing, presentation, file sharing and spreadsheet programs.
    • Ability to maintain and adhere to strict confidentiality policies.
    • Ability to travel in state vehicle and in personal and rental vehicles.
    • Ability to manage detailed confidential records.
    • Ability to prepare concise, accurate reports.
    • Ability to handle themselves professionally under stressful situations.
    • Ability to work as a team member.
    • Ability to travel 35% of the time.
    • Ability to obtain a CPR certification within the first six months of employment.
    • Ability to receive Child Passenger Safety Certification within one year of employment.
