    Responsibilities

    • Work as a part of the CE team to continue to build an effective CE process for the Nashville-Davidson County Continuum of Care meeting federal requirements
    • Assist in the implementation of special projects such as the WeGo bus pass program, called Drive to End Chronic Homelessness
    • Assist in the documentation and processing of referrals made through the CE process
    • Provide community outreach and education to partner agencies regarding CE process
    • Focus on assisting staff members at community coordinated entry access points to ensure people are served in an equitable and open manner.
    • Provide CE access and assessment to those who are in a housing crisis and/or experiencing literal homelessness and provide basic resources and referrals.
    • Assist those who are experiencing literal homelessness in obtaining vital records and other documentation needed for housing
    • Enter timely data into the Homeless Management Information System (HMIS) database for the purpose of CE access, assessment and referral processing
    • Attend related community meetings and participate in weekly Care Coordination meetings
    • Support the work of the CE manager and CE Special Projects Coordinator
