    This position will involve visits in the West Valley (Laveen, Tolleson, Phoenix, Glendale) from Monday to Friday, 8am - 5pm.

    As a Physical Therapist at Banner Home Health, your role includes evaluation, treatment planning, and supervision according to established standards and policies. Your competence is crucial in providing patient-centered care.

    CORE FUNCTIONS
    - Evaluate patient's abilities and limitations, establish treatment goals
    - Implement personalized treatment plans considering patient's needs
    - Document treatment in a timely and skilled manner
    - Communicate effectively with healthcare team and patients/families
    - Demonstrate adaptability and supervision in patient care
    - Engage in departmental activities and process improvement
    - Interact with various stakeholders concerning patient care

    MINIMUM QUALIFICATIONS
    - Graduate from an APTA accredited Physical Therapy school
    - Possess valid Physical Therapy license and BLS certification
    - Demonstrate clinical competency and relevant experience

    PREFERRED QUALIFICATIONS
    - Additional related education/experience is advantageous
