    Job Description

    Under the direction of the Director of Clinical Services, the Licensed Practical Nurse (LPN) is responsible for providing quality care to residents of assigned neighborhood. The LPN functions as a professional caregiver who is accountable for planning, implementing and evaluating all aspects of resident care, including physical, psychosocial and environmental needs. Performs all job responsibilities according to Duncaster's Policies & Procedures.

    Job Duties

    • Contributes to the achievement of excellence through the quality and care values of the mission of Duncaster.
    • Administers medications accurately and in a timely fashion.
    • Documents according to nursing practice standards and Duncaster policies and procedures.
    • Provides treatments to residents according to physician orders and Duncaster policies and procedures.
    • Provides for physical comfort and a safe environment.
    • Follows infection control, bloodbourne pathogens and hazardous materials policies and procedures.
    • Protects residents' rights, confidentiality and dignity.
    • Accurately carries out physician/provider orders in a timely fashion.
    • Initiates, implements and evaluates care plans/MDS on an ongoing basis.
    • Accurately completes admission and discharge procedures.
    • Coordinates staff and resident care appropriately to meet resident needs.
    • Willingly participates in organizational/professional activities.

    Education and Experience

    • Requires licensing as a Licensed Practical Nurse licensed to practice in the State of Connecticut.
    • Comprehensive knowledge of geriatric nursing care.
    • Comprehensive knowledge of principles of nursing, and physical evaluation skills.
    • Basic knowledge of human relations skills necessary to deal effectively with the geriatric residents, their families, and CHHC staff.
    • A minimum of two years of full-time clinical experience in nursing.

    Hours

    Per Diem and Flex Schedules Available

    1st: 7:00am-3:30pm

    2nd: 3:00pm - 11:30pm

    3rd: 11:00pm - 7:30am

    Pay Rate

    Per Diem

    $30.00 p/h + shift differentials ($2.00 p/h weekday evenings, nights & weekend days, $3.00 p/h weekend evenings and weekend nights)

    Flex

    $35.00 p/h (must have 3 years of skilled nursing experience) + shift differentials ($2.00 p/h weekday evenings, nights & weekend days, $3.00 p/h weekend evenings and weekend nights)
