Location: Tolland, CT 06084
Salary: $40 - $50 per hour

Responsibilities:

• Perform comprehensive patient assessments and develop care plans.
• Administer medications and conduct treatment as prescribed.
• Monitor patient conditions and adjust care plans as necessary.
• Maintain precise medical records and documentation.
• Work collaboratively with healthcare professionals to ensure complete patient care.
• Educate patients and their families on health conditions and care requirements.

Qualifications:

• Nursing degree with active RN or LPN license in Connecticut.
• Current CPR/BLS certification.
• Prior home health experience is a plus.
