    SUMMARY DESCRIPTION OF WORK

    This intern will serve as a direct care staff/case coordinator for the CCBHC program and/or the Peer Support Center located onsite. This position provides supervision and support to clients as a part of the CCBHC-NE Program, as well as case coordinator services for assigned clients. The Specialist will be part of the team that provides education and supportive programming for men and/or women, their children and family members if included. The CCBHC-NE also services children and youth.

    Entry level professional position. Educational requirements are specific. Decision making skills and problem-solving skills are required.

    Work Schedule: Monday through Friday, 8:30 a.m. and 5:00 p.m. (30 hours per week)

    MINIMUM QUALIFICATIONS

    • Undergraduate Internship/Practicum level student/Foundation student

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    • *Provides supervision and support for program participants as a part of a case coordination team.
    • *Documents client activity, progress and needs per established record keeping procedures.
    • *Documents and completes program assessments, including though not limited to NOMs, PHQ-9, Columbia Screener and substance and nicotine use screeners.
    • *Participates in interdisciplinary client/family/case staffing meetings to obtain information, provide information, and assist with service planning.
    • *Implements case coordination plans and provides support services for program participants under the direction of Behavioral Health Navigator and Project Director.
    • *Does follow-up with individual clients as assigned and updates goal planning as client goals change.
    • *Responsible for maintenance and upkeep of facility, including monitoring clients' activities.
    • *Orients clients to program rules and assists with creation and implementation of Peer Support Center as needed.
    • Works as a team member with other program staff to provide trauma-informed care to clients.
    • *Researches, plans, assists, and provides behavioral health, recreation, and/or informational classes in a group setting and in one-to-one interaction with clients.
    • Networks with community providers and other agency programs to assist clients in accessing needed community resources.
    • *Remains knowledgeable of support groups that provide supportive services for persons with substance use and/or mental health issues and their families.
    • Evaluates efficiency and effectiveness of community service providers on an ongoing basis to ensure clients are receiving quality care.
    • *Develops supportive relationship with clients.
    • Advocates for clients in the community.
    • Maintains required records and statistical information.
    • Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.
    • Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
    • Is dependable and punctual regarding scheduling and attendance.
    • Abides by all specific program and Agency procedures, policies, and requirements.
    • Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
    • Performs other program related duties as assigned.
