    Job DescriptionTherapy at Home A Private Practice setting is looking for a Physical Therapist. Territory to cover: Menomonee Falls and Northwest side of Milwaukee. New graduates encouraged to apply

    Job Description:
    We would love to meet you and introduce you to our exceptional team and unique approach, whether you are an experienced PT or someone seeking mentorship. Our focus is on providing care for the elderly in settings such as ALF, Memory Care, Home, Group Home, and Day Centers. (This is a mobile position providing care in their homes.)

    Responsibilities:
    • Perform evaluations and develop treatment plans
    • Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
    • Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
    • Communicate patients needs and progress to the treatment team, physician, person receiving services and family members
    • Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes
    • Supervise Physical Therapist Assistants as needed.
    Qualifications:
    • Minimum of a Bachelor's Degree from an accredited Physical Therapy program
    • Valid Physical Therapist license in Wisconsin
    • A minimum of one years work experience as a Physical Therapist is preferred, but not required.
    • Experience with dementia is preferred.
    Compensation: Starting $80,000 and up
