JOB SUMMARY:
This role involves providing peer support services with a rehabilitation and recovery focus to individuals coping with behavioral health symptoms. Activities include advocacy assistance seeking benefits entitlements food shelter permanent housing linkages to systems navigation within behavioral health allied human services systems access appropriate care outreach engagement companionship modeling recovery lifestyle raising awareness existing services pathways recovery helping person remove barriers access them interim visits individuals families discharge hospital emergency rooms detox units inpatient psychiatric units face-to-face service increasing motivation participate clinically indicated treatment chemical dependence.
