- Provides care consultation services to individuals with dementia (any stage) and their care partners. Care consultation may consist of assessment of needs, identification of issues/concerns and available resources, development of a plan of care, referral to appropriate resources, provision of emotional support and education about dementia, and follow-up, as needed. Care consultation services may be provided by phone, virtually or in person through individual and family meetings.
- Participates in regular clinical staffing, group/individual supervision, and continuing education sessions and discusses challenges cases with supervisor, as appropriate.
- Develop & implement a data-informed, community impact plan to expand reach in each assigned market of opportunity defined by a strategic geography or population.
- Conduct ongoing community assessments through Community Forums in each market to ensure that volunteer-led programs are aligned with community needs and offered where demand is high.
- Recruit, train and lead volunteers to deliver and grow programs in designated markets. Provide ongoing technical assistance, coaching and support. Ensure accountability by regularly monitoring volunteer impact and taking action to improve volunteer performance.
- Manage and maintain a portfolio of high-impact community partners using a relationship sales approach to grow the Association's program reach. Expand collaborative agreements with current partners in community networks, organizations, service providers, nonprofits and faith institutions, and establish new community partnerships beyond traditional partners to reach underrepresented communities.
- Create urgency for our cause by promoting a full-mission value proposition to support Alzheimer's Association campaigns such as quality care, fundraising and advocacy efforts throughout designated communities.
- Model and foster behaviors that contribute to a culture that values the staff/volunteer/ community partnership and is consistent with the Association's goals, best practices, and core values.
- Ensure timely and accurate reporting of program, community impact and volunteer performance data as well as grant-funded projects (where applicable).
- Partners with all mission pillars (Public Policy, Development, Concern and Awareness, Health Systems, Operations to advance internal key performance indicators. Examples may include: recruiting advocates, providing media interviews, and advancing the Association's health systems strategy).
- Fully integrates DEI activities into all programs and services.
- Provides backup support to ND Program Director and other staff, as needed with all activities.
- Travels to communities in western North Dakota to deliver education, care consultation, and implement program reach strategy, whenever virtual delivery is not possible.
- Other duties as assigned.
- Minimum four-year degree in social work, nursing, counseling, education, occupational therapy, psychology, sociology, therapeutic recreation, or related human service field is required. A Master's degree in counseling, social work, nursing, or psychology is preferred, or Master's degree equivalent experience, especially in the areas of dementia care or elder services may be considered
- 1-2 years of experience working in the geriatric field and/or the provision of psycho-social assessments and related action planning
- Experienced clinical/counseling skills in listening, assessment, empathy, advocacy, negotiation, referral, and closure
- Experience working with diverse communities preferred
- Professional experience conducting health evaluations, diagnostic intakes, and/or crisis intervention counseling
- Strong presentation skills and previous experience as a public speaker or educator
- Proven success in building and maintaining key relationships and meeting strategic goals through volunteer-led initiatives.
- Able to identify and cultivate relationships with community partners, organizations, volunteers and diverse populations to create sense of urgency related to dementia awareness as a major health issue and motivate all to action.
- Ability to effectively collaborate with others, hold others accountable and reach mutually agreeable outcomes.
- Familiarity with a public health approach focused on health promotion and early detection.
- Experience building rapport/trust, assessing needs and articulating a value proposition.
- Ability to represent the Association effectively to community partners, community leaders, corporate partners.
- Excellent written and presentation skills
- General knowledge of community health, community-based organizations and systems.
- Knowledge of Alzheimer's disease and related disorders preferred
- Strong organizational, staff and project management skills
- Knowledge of community resources
- Ability to communicate effectively, both orally and in writing, with attention to detail
- Ability to work in a fast-paced environment and meet deadlines with flexibility and good humor
- Willingness to work on evenings and weekends, if necessary

Description
The purpose of this role is to implement the Alzheimer's Association's program delivery and growth strategy to reach more people with volunteer-delivered programs, especially in new, underrepresented and rural communities. The Program Manager is responsible for identifying, recruiting, engaging and growing volunteers and community partners while meeting Association best practices to achieve growth goals and benchmarks. This position will partner with all other mission pillars of the Alzheimer's Association (Public Policy, Development, Concern and Awareness, Health Systems and Operations) to assist in the strategic goals in all areas. The position is responsible to serve designated counties in Northwestern and Central North Dakota, including the follow counties; Divide, Williams, McKenzie, Burke, Mountrail, Dunn, Renville, Ward, McLean, Bottineau, McHenry and Pierce, however may provide services in other areas of ND as needed. This position is partially grant funded and will help to fulfill requirements of a contract with the North Dakota Department of Health and Human Services, Aging Services Section including the facilitation of care consultation and education, accurate and timely documentation, processing referrals and collaborating with other organizations across ND.
