RN Home Health Admissions Nurse
PRN Weekend Schedule

A Home Health Admissions Nurse is primarily responsible for performing the admission visit for home health patients referred to our service.

The Admissions nurse must have a good understanding of OASIS, and have good assessment skills that enable them to recognize the need for other disciplines that may be necessary on a case and not yet ordered by the physician.

This applicant must possess excellent communication and organizational skills.

The duties of an RN as stated below also apply; however, the role of this nurse is to work to admit patients.

The Registered Nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on health education/experience.

The nurse builds from the resources of the organization to plan and direct services to meet the needs of individuals and families within their home.

Skills and Experience

A successful admission nurse will be skilled at "processing" patients - that is, working with one patient and then sending that patient off to another nurse's care.

Admissions areas are often fast-paced and require good judgment to determine the initial course of the patient's care.

Must be able to perform assessment/data collection in manner that includes mental and cognitive status, family/social /developmental issues and functional ability.

One year of RN experience, Home Health experience preferred.
CPR Certification
Valid driver's license and reliable transportation required.
