    A health services agency in New York City is currently seeking a licensed Registered Nurse (RN), In this role, the Registered Nurse (RN) is responsible for providing direct nursing care to agency clients in accordance with agency policies and procedures and current standards of Nursing Practice. Completes assigned duties and documents in the medical record in accordance with medical record policies and procedures.

    About the Opportunity:

    • Schedule: Monday to Friday
    • Hours: 9am to 5pm or 10am to 6pm
    • Setting: Outpatient
    • Program: Independence Program
    • Patient Population: Men & Women struggling with addiction, military veterans, mothers and babies, families, homeless New Yorkers, and the elderly

    Responsibilities:

    The Registered Nurse (RN) will:

    • Assess health status of clients assigned to daily work schedule and develop a plan of care
    • Perform and document nursing intake / medical history in accordance with agency policy and procedure
    • Triage sick call list as assigned and make referrals for treatment to appropriate level of medical practitioner
    • Complete follow-up assessment of clients who are treated for chronic and/or acute illnesses to monitor effectiveness of prescribed treatments
    • Review medical record to assess care being provided and to ensure continuity of care
    • Monitor vital signs and performs physical examination, as indicated to assess health status of clients
    • Develop a plan of care for nursing care in accordance with standards of practice
    • Document doses administered or packaged on the medication administration record
    • Document all assessments, interventions and response to treatments in the progress notes
    • Prepare medication doses for direct administration to agency clients during clinic hours of operation
    • Provide patient education relative to diet, exercise, medication and treatments that are ordered
    • Maintain inventories of stock items and order supplies as indicated
    • Perform other duties, as requested

    Qualifications:

    • 1+ year of Nursing experience
    • Associate's Degree in Nursing
    • NYS Registered Nurse (RN) licensure
    • BLS certification
    • Computer savvy
    • Knowledge of medical terminology
    • Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds
    • Knowledge of techniques for identifying and preventing potentially violent behavior, including crisis management techniques
    • Knowledgeable about federal, state and local law and regulation governing substance abuse treatment programs
    • Solid assessment, clinical ,and documentation skills

    Desired Skills:

    • Bachelor's Degree is Nursing (BSN)
    • Experience working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI)
