- Responsible for on-call services, as assigned.
- Maintains records associated with Hospice service benefits: medical equipment and supplies, pharmaceuticals, levels of service, and coordinates with the appropriate vendor or billing personnel when needed.
- Assists with the referral/ admission/ discharge process flows as it relates to medical records and hospice care and services.
- Work cooperatively and professionally with Hospice Director, Patient Care Coordinators and hospice staff in identifying and recruiting for Volunteer needs.
- Maintains volunteer records including but not limited to; patient visits, recruitment, onboarding, recognition and retention efforts, volunteer hours, etc.
- Works collaboratively with care teams regarding volunteer assignments.
- Maintain and support office operations including office supplies and forms, admission packets, forms, and computer/ technology support.
- Performs other similar duties, as needed.
- Commitment to quality outcomes and services for all individuals.
- Ability to relate well to all individuals.
- Ability to maintain and protect the confidentiality of information.
- Ability to exercise independent judgment and make sound decisions.
- Ability to adapt to change.
- High School diploma or equivalent required.
- Education in office administration preferred.
- One year of experience in a related field.
- 2-3 years of experience with volunteer services preferred.
- Excellent working knowledge of Microsoft suite of platforms.
