    Summary:


    The Director of Clinical Integration plays a critical role in enhancing coordination and integration within Friend Health (FQHC) and HRDI.

    This position focuses on management of integrative processes within clinical areas in FQHC and bridging the gaps between two organizations to ensure seamless patient care, improve patient outcomes, and promote a culture of quality care.

    The Director will also be responsible for understanding key performance indicators (KPIs), output vs. outcome, proactive community needs assessment, staff management, knowledge building, and aligning with organizational goals.


    Key Responsibilities:

    Integration and Care Coordination:
    Collaborate with Friend Health (FQHC) and HRDI teams to develop effective care integration strategies.
    Facilitate the coordination of care for patients with behavioral health conditions, substance use disorders, and other chronic conditions.
    Ensure smooth transitions of care with warm handoffs and follow-through for additional services.
    Establish and maintain care coordination processes to bridge organizational gaps.
    Oversee the requirement for FQHC physical examinations for patients entering HRDI Behavioral Health programming.

    Foster a culture of integration across all healthcare teams within thecombinedorganization, promoting collaboration and cross-functional communication to improve patient care.


    Population Health Management:
    Manage the behavioral health population, with a focus on dual-diagnosed patients, Infectious Disease and chronic conditions.
    Utilize data analysis to identify individuals with high admission rates and low follow-up rates, implementing targeted interventions.
    Collaborate with behavioral health and primary care teams to improve care for dual-diagnosed patients.
    Continuously assess the needs of the population to proactively prepare for community healthcare requirements.


    Department Building:
    Lead the building of Integrated Care Coordination department.
    Implement a risk stratified caseload system based on patient needs, providers, and services.
    Collaborate to enhance care coordination, especially for non-county care patients.
    Foster a culture of care coordination across all departments.


    Quality Improvement:
    Work with both organizations to update workflows, enhance competency, and promote evidence-based practices.
    Collaborate with the quality department to produce reports, analyze data, and address operational gaps.
    Develop strategies to improve adherence to clinical guidelines, enhance quality of care, and improve outcomes.
    Lead efforts to integrate quality operations and provide data-driven insights to improve the quality of care.

    Understanding KPIs and Outcome Measures:
    Develop a deep understanding of key performance indicators (KPIs) and the distinction between output and outcome measures.
    Use this understanding to drive data-driven decision-making and improve the effectiveness of care coordination efforts.

    Staff Management and Knowledge Building:
    Effectively manage and lead the Integrated Care Coordination department, fostering a culture of continuous learning and development.
    Support staff in building their knowledge and competencies to enhance care coordination processes.
    Develop and implement efficient and effective workflows for combined organization.
    Supervise Integrated Care Coordinator(s) at HRDI and Lead BH provider at FH


    Proactive Community Needs Assessment:
    Stay proactive in assessing the evolving healthcare needs of the community and adapting integration and practice management strategies accordingly.


    Alignment with Organizational Goals:


    Ensure that all integration and practice management efforts align with the overarching goals and objectives of Friend Health and HRDI.


    Qualifications:
    Master's degreerequired in healthcare related field of study.
    Minimum 10years of healthcare management experience, with expertise in care coordination and integration.
    Knowledge of behavioral health and population health management.
    Strong analytical skills and proficiency in data analysis tools.
    Strong ability to use Electronic Health Record and other Population Health Management Software
    Exceptional leadership and team management abilities.
    Effective communication and collaboration skills.
    Understanding of healthcare billing and quality improvement processes.
