- Case manages clients in the community under program contracts.
- Provides clinical consultation to licensed social workers and patient care team to facilitate coordination of care across the continuum for qualified clients.
- Key responsibilities include, but are not limited to, case finding and assessment, patient/family education and advocacy, resource management, service planning, service coordination and monitoring.
- Essential: lifting up to 2 lbs., seeing, hearing, talking, grasping, fingering.
- Continuous: standing, walking, stooping, forward reaching, handling.
- Frequent: pushing and pulling up to 50 pounds of force, lifting up to 10 lbs., sitting, bending, squatting, twisting.
- Occasional: pushing and pulling up to 50 pounds of force; lifting: horizontal lift/carry up to 50 pounds, floor to waist up to 35 lbs., waist to shoulder up to 25 lbs., overhead level up to 15 lbs.; carrying up to 35 lbs, running, climbing, side bending, kneeling, crouching, overhead reaching.
- Operates various machines/tools/work aids.
- Office and community work.
- May be exposed to body fluids, blood products, and communicable diseases.
- May be subjected to clients with behavior health conditions.
- Work environment may be highly stressful, fast paced and hectic.
- Access to a motor vehicle is required.
- Current Hawaii state license as a Registered Nurse.
- Bachelor's degree in nursing.
- Valid U.S. driver's license required in accordance with HI State law rules and regulations.
- Meet and comply with qualifications set forth in QMC Fleet Safety Policy.
- Driver's abstract showing evidence of good driving record.
- Current certification in CPR at BLS/HCP level.
- Two (2) years nursing experience preferably with a mixture of acute and outpatient setting.
- Prior community case management experience preferred.
- Prior clinical and/or ICU/ER experience preferred.
- Mental health or psychiatric experience desirable.
