    RESPONSIBILITIES

    JOB SUMMARY/
    RESPONSIBILITIES:


    • Case manages clients in the community under program contracts.
    • Provides clinical consultation to licensed social workers and patient care team to facilitate coordination of care across the continuum for qualified clients.
    • Key responsibilities include, but are not limited to, case finding and assessment, patient/family education and advocacy, resource management, service planning, service coordination and monitoring.

    TYPICAL PHYSICAL DEMANDS:

    • Essential: lifting up to 2 lbs., seeing, hearing, talking, grasping, fingering.
    • Continuous: standing, walking, stooping, forward reaching, handling.
    • Frequent: pushing and pulling up to 50 pounds of force, lifting up to 10 lbs., sitting, bending, squatting, twisting.
    • Occasional: pushing and pulling up to 50 pounds of force; lifting: horizontal lift/carry up to 50 pounds, floor to waist up to 35 lbs., waist to shoulder up to 25 lbs., overhead level up to 15 lbs.; carrying up to 35 lbs, running, climbing, side bending, kneeling, crouching, overhead reaching.
    • Operates various machines/tools/work aids.

    TYPICAL WORKING CONDITIONS:

    • Office and community work.
    • May be exposed to body fluids, blood products, and communicable diseases.
    • May be subjected to clients with behavior health conditions.
    • Work environment may be highly stressful, fast paced and hectic.
    • Access to a motor vehicle is required.

    MINIMUM QUALIFICATIONS:
    A


    EDUCATION/CERTIFICATION AND LICENSURE:

    • Current Hawaii state license as a Registered Nurse.
    • Bachelor's degree in nursing.
    • Valid U.S. driver's license required in accordance with HI State law rules and regulations.
    • Meet and comply with qualifications set forth in QMC Fleet Safety Policy.
    • Driver's abstract showing evidence of good driving record.
    • Current certification in CPR at BLS/HCP level.

    EXPERIENCE:

    • Two (2) years nursing experience preferably with a mixture of acute and outpatient setting.
    • Prior community case management experience preferred.
    • Prior clinical and/or ICU/ER experience preferred.
    • Mental health or psychiatric experience desirable.
