    Desk Nurse- LPN (Licensed Practical Nurse)

    Claremont Village ALP is looking for an experienced LPN Desk Nurse to join our assisted living program in Bronx, NY. The ideal candidate should have prior desk nurse experience and a positive team-oriented attitude.

    DUTIES:
    • Follow all NYSDOH regulations.
    • Provide leadership and guidance to staff to meet organizational goals.
    • Maintain a clean work environment.
    • Supervise Medication technicians regularly.
    • Manage medication orders and monitoring for residents.
    • Assist residents with medication administration following protocols.
    • Ensure proper disposal of discontinued medications.
    • Handle medication orders and renewals promptly and accurately.
    • Check medication refill log against storage cabinets inventory.
    • Address 'no-refill' orders and request new orders from MDs.
    • Manage receipt, storage, and documentation of delivered medications.
    • Keep medical and medication records updated for all residents.
    • Handle medication paperwork for admissions and changes.
    • Conduct medication room audits for compliance.
    • Verify consistency in prescriptions, blister packs, and administered medications.
    • Communicate with physicians and coordinate orders effectively.
    • Arrange lab draws for residents as per orders.
    • Review and follow up on received lab results with physicians.
    • Ensure regular weight checks are done, documented, and communicated to physicians.
    • Provide assistance in medical or mental health emergencies as necessary.
    REQUIREMENTS:
    • Active LPN License.
    • 3-5 years of preferred experience in Long Term Care.
    • Basic computer proficiency.
    • Strong communication skills and ability to work well within a team.
