    Job Requirements


    Education Requirements:
    Valid registered nurse (RN) license in the state of Minnesota


    Certifications Requirements:
    Current BLS certification; CPR certification; Must possess MDS certification or relevant coursework


    Experience Requirements:
    Minimum of 1 year of experience in skilled nursing facilities; Prior experience as an MDS coordinator; Familiarity with Medicare and Medicaid guidelines


    Other Requirements:
    Ability to work 5x8 schedules Monday through Friday and every third weekend; Strong organizational and communication skills; Ability to adapt to EMR systems and maintain compliance; Professional appearance in scrubs (if required); Flexibility and reliability

    Job Responsibilities

    Provide comprehensive MDS assessments adhering to federal and state regulations to ensure accurate resident classification and reimbursement.

    Collaborate with interdisciplinary teams to gather necessary clinical documentation and support quality care delivery.

    Maintain and update resident records within EMR systems, ensuring all data is accurate and compliant with organizational policies.

    Execute audits and quality assurance processes related to MDS to optimize facility accuracy and reimbursement rates.

    Educate staff on MDS requirements and processes to promote compliance and improve documentation practices.
