    Position Summary


    Shift: 8a-5p M-F and On- Call Rotation

    Responsible for establishing the standards of nursing care and clinical competencies to meet patient needs and prepares and delivers training programs to ensure clinical expertise

    Essential Duties and Responsibilities


    · Provides consultation for complex wound, ostomy, and incontinence patients according to referral needs

    · Takes leadership role in development, coordination, and monitoring of agency's wound care program and monitors and works to improve clinical outcomes

    · Adheres to wound care policies and procedures to include patient care protocols, documentation tools, and wound care formulary and facility referral criteria

    · Provides basic wound care services, keeps other licensed clinical personnel informed on items related to wound care, ostomies, and incontinence

    · Makes recommendation to the physician for changes to wound care orders and provides evidence based research support as needed

    · Completes/updates the Electronic documentation as required

    · Maintains and updates Nursing license in compliance with state regulations

    · Performs other related duties as required or requested

    Minimum Qualifications (Knowledge, Skills, and Abilities)


    · College degree or the equivalent in work related experience

    · Must be graduate of an accredited school of Licensed Vocational Nursing

    · Must have active state LVN license

    · Must be listed in good standing as confirmed on Nurse Registry

    · Strong knowledge of state, federal, and local regulations as they pertain to long term care is strongly preferred

    · Must be able to communicate verbally and in writing in English

    Physical Demands – Office

    While performing the duties of this position, the employee is regularly required to talk or hear.     Work Environment – Office

    This job operates in a professional office environment with minimal exposure to dust, chemicals or noise. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
