- Conduct initial assessments for all new participants and perform quarterly follow-up examinations during their first year of enrollment.
- Assess participants for episodic visits, acute illnesses, and follow-ups after hospitalizations.
- Conduct nursing home visits as per state requirements and when necessary.
- Provide home visits for acute or chronic situations when participants are unable to leave their homes.
- Participate in After-Hours On-Call Coverage.
- Deliver relevant health education to patients, families, caregivers, and other team members.
- Follow the standards outlined in the Element Care NP/MD Collaborative Practice Guide.
- Ensure accurate and timely documentation, both onsite and offsite.
- Perform all duties in a financially responsible manner within established budgetary limits.
- Conduct client interactions with the highest standards of care.
- Must pass a fit test and wear a mask when a tight seal is crucial. No facial hair should obstruct the respirator seal.
- Requires frequent local travel.
- Other duties as assigned.
- Master of Science in Nursing (MSN) degree.
- Current Nurse Practitioner board certification in Massachusetts.
- Massachusetts and DEA Prescription Privileges.
- Credentialing for Element Care affiliate Long Term Care Facilities.
- Current CPR Certification.
- Minimum of one year of experience working with geriatric populations.
- Ability to communicate information clearly and concisely to all staff members promptly.
- Demonstrated independence and strong teamwork skills.
- Detail-oriented, self-motivated, and able to meet deadlines.
- Quick problem recognition and resolution abilities with an understanding of consequences.

The Nurse Practitioner plays a key role as part of the PACE Interdisciplinary Team, contributing to the planning, execution, and evaluation of care plans in alignment with the goals, standards, and policies of the PACE model. Collaborating closely with Medical Doctors, the Nurse Practitioner delivers comprehensive primary care to program participants. This is a full-time role, from Monday to Friday, 8 AM to 4 PM, with no night shifts, weekends, or holidays.
