Job Summary:

The Community Outreach Coordinator is responsible for leading day-to-day community implementation across multiple, interconnected health equity grants, including the HEAL Collaborative and BARR Foundation initiatives. Working in close collaboration with the Program Manager, institutional partners, and resident leaders, this role is responsible for coordinating outreach, engagement, planning, implementation, and reporting activities across several North Central Massachusetts communities.

The position centers resident leadership, with a strong focus on individuals and communities that have been historically marginalized and are most impacted by social determinants of health. The Coordinator plays a key role in building resident power, strengthening cross-sector partnerships, and translating community-identified priorities into actionable, equitable strategies related to healthy food access, economic empowerment, and social inclusion.

Responsibilities

Essential Functions

  • Work in close collaboration with the Program Manager to plan, implement, and continuously improve community-based initiatives for the HEAL and BARR grants.
  • Lead and coordinate day-to-day implementation of grant activities for HEAL and BARR, managing timelines, work plans, deliverables, and milestones across multiple communities.
  • Support planning and execution of resident-led and partner-supported community projects addressing healthy food access, economic empowerment, and social inclusion for both grants.
  • Recruit, train, coordinate, and support resident leaders across all HEAL and BARR communities, ensuring representation of diverse populations and lived experiences.
  • Facilitate resident leadership meetings, goal-setting sessions, and action-planning workshops for HEAL and BARR resident leaders.
  • Provide ongoing coaching, technical assistance, and follow-up to resident leaders to support implementation of community-identified priorities under both grants.
  • Build strong, trusting relationships with residents and youth to support leadership development and participation in decision-making spaces for HEAL and BARR initiatives.
  • Ensure resident-led strategies are implemented equitably and remain aligned with community-identified priorities across all grant activities.
  • Design, coordinate, and implement outreach strategies to engage a representative cross-section of residents for HEAL and BARR, using tools such as community network analysis, listening sessions, surveys, and direct engagement.
  • Conduct community needs assessments and root-cause analyses to inform local and regional action planning under both grants.
  • Organize and facilitate community meetings, forums, events, and regional convenings with residents and partners for HEAL and BARR projects.
  • Establish, maintain, and grow strong working relationships with institutional partners, municipal departments, nonprofits, and community organizations across all grant communities.
  • Convene and facilitate regular partner meetings and cross-community coordination sessions to align efforts, share progress, and address challenges for HEAL and BARR.
  • Coordinate partner roles and responsibilities to support resident-led initiatives and avoid duplication of services across grants.
  • Build and strengthen networks of wrap-around community support resources responsive to resident-identified needs for HEAL and BARR communities.
  • Track participation, activities, and outcomes across all HEAL and BARR grant activities.
  • Collect, manage, and submit required data and documentation; complete monthly grant reports and support evaluation efforts for both grants.
  • Attend relevant trainings, conferences, and community events to strengthen partnerships, build capacity, and support continuous improvement across HEAL and BARR work.

Physical Requirements:

Minimum Work Experience

  • 1-2 years of previous experience in a community or hospital outreach role with supervisory responsibilities necessary.

Minimum Licenses and Certifications

  • Trained DiSC certified
  • Training Active Bystanders
  • Community Health Worker or equivalent hours
  • DTA and SNAP registered a plus.

Required Skills

  • Ability to communicate effectively both orally and written.
  • Strong interpersonal skills, tact, professional appearance, and ability to make decisions and supervise others.
