- Collaborate with Atria physicians to deliver exceptional care both at our state-of-the-art facility and in members' homes
- Work with CMOs on Preventive Care, Well Visits & Comprehensive Health Assessments, and provide supervision for Home Care visits
- Be willing to work weekends and off hours for on-call coverage as necessary
- Ability to work across various disciplines such as Internal Medicine, Cardiology, Neurology, and Geriatrics for holistic care
- Provide age-appropriate care to members
- Training in various Point of Care Testing modalities and devices
- Perform clinical, administrative, and clerical tasks as needed
- Deliver Home Care Services including minor procedures and medication administration
- Maintain accurate Medical Records for all members
- Conduct Telehealth visits and Electronic prescribing
- Participate in organizational initiatives for Quality & Patient Safety
- Ensure compliance with licensing and certification requirements
- Engage in continuing medical education programs
- Opportunities for professional growth and development
- Current NP or PA License
- Minimum 2 years of NP or PA practice experience
- ACLS, PALS, and BLS certification
- Proficient in urgent care and emergency medicine procedures
- Availability for weekend on-call shifts
- Ability to thrive in a dynamic and fast-paced environment
