- You assist the physician in evaluating the functioning level in each patient by providing initial and ongoing assessments.
- After deciding based on assessments, you aid in developing or revising the plan of care, prepare clinical and progress notes, and consult with the family and other agency personnel.
- You then provide the suitable therapeutic treatments while ensuring the comfort of your patients.

- You deliver all services following CMS documentation and billing requirements, including functional reassessment requirements.

- You also promptly identify and report any instances of abuse in accordance with the policy as well as federal and state laws.
- When necessary, you aid in onboarding for new hires and participate in training activities.

- Successful completion of a physical therapy education program
- Current license in good standing with the state of Oklahoma medical board
