    Responsibilities:

    The primary purpose of the Social Worker is to oversee case management and social support for residents within the nursing home on the long term care units. The Social Worker will provide quality psychosocial services to foster residents wellbeing and maximum independent functioning within the nursing home and as needed at the adult day care setting. The Social Worker supports the residents, staff and families with the problems that delay or prevent the residents social adjustment or any other problem that affects their ability to benefit from their stay in the facility.

    • Identifies and addresses case management needs, makes referrals, recommendations, behavioral interventions.
    • Complete social histories.
    • Completes admission protocol, inclusive of all admission paperwork and policies with resident or sponsor.
    • Updates/creates cognitive, behavioral, mood, advanced directive and discharge planning sections in care plan and MDS.
    • Responsible for Resident right/quality of life reviews.
    • Responsible for assisting residents with Advanced Directive decisions and paper work.
    • Assures this documentation is filed in the chart and EMR.
    • Responsible for coordinating virtual psychiatric visits if necessary.
    • Assists with coordinating safe discharges to the community when recommended.
    • Provides 1:1 and other behavioral interventions as needed.
    • Understands pre-admission and admission process for possible coverage in the absence of the Director and Assistant Director.
    • Responsible for coverage of the Nursing Home and offsite Adult Day Care Program.
    Qualifications:

    Education:

    • 4 year/Bachelor' degree in social work, psychology, sociology or related field, required

    Experience:

    • Minimum one year experience in a social services setting, required
    • Minimum one year in a long care term setting, preferred

    Physical Requirements:

    • Light work - Exerting up to 20 lbs of force frequently, and/or a negligible amount of force constantly to move objects.

    • Frequent typing/clerical/dexterity, hearing, seeing/monitor/computer use, standing, talking, visual acuity (color, depth, perception),walking

    Hazards:

    • Occasional bodily fluids/bloodborne exposure

    Working Conditions:

    • Occasional noise, odors

    Travel:

    • Occasional travel the job may require travel from time-to-time, but not on a regular basis

    Sign-on Bonus:
    USD $20,000.00

    Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. The above compensation range represents a good faith belief of the compensation range by Bassett Healthcare Network, and its entities and affiliates, at the time of this posting or advertisement.

    Min:
    USD $22.00/Hr.

    Max:
    USD $26.00/Hr.
