    Faith Presbyterian Hospice

    Hospice Aide (CNA) - Inpatient

    7am - 7pm

    7pm - 7am

    Responsibilities:

    ESSENTIAL DUTIES:
    Models a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers
    Provides all personal care/homemaking services according to the plan of care under the supervision of a Registered Nurse
    Practices Universal Precautions according to OSHA regulations and company policy
    Maintains confidentiality of all patient information, sharing only what needs to be shared with the interdisciplinary team in order for the patient to be adequately care for
    Provides or assists in feeding, bathing, dressing, grooming, toileting and/or showering, while ensuring privacy and safety and encouraging independence
    Makes beds, straightens rooms and keeps patient/family personal areas in an organized and clean manner
    Respects patient/family rights by: (knocking on doors before entering the residence, addresses patient/family by name, allows patient to make own decisions when appropriate)
    Provides emotional support and compassionate care to the patient and family. Demonstrates listening skills, offers encouragement when appropriate, adjusts care to the patients physical condition each visit (i.e., allowing patient rest periods during personal care)
    Provides safe patient transfers involving bed, wheelchair and/or shower chair, uses the gait belt with all transfers, to protect patients
    Takes and records temperature, pulse, blood pressure and respirations as required
    Assists patients with simple range of motion or other exercises as directed by nurse or therapist and ambulates patient safely according to his/her endurance levels
    Applies medical or adaptive devices as ordered by the appropriate supervising nurse
    Accurately documents care given to patients including the documentation of reportable patient problems (i.e., fever, constipation, s/s of skin breakdown, etc.).
    Reports changes in patient condition to the supervising nurse and/or primary RN case manager. Reports problems with completion of assignments
    Completes required documentation prior to end of shift
    Handles confidential information appropriately
    Demonstrates flexibility with changing work demands
    Reports to supervising nurse and/or primary RN case manager: equipment and supply needs, repair/maintenance needs, safety hazards and unusual occurrences or observations
    Attends and participates in appropriate in-service and staff meetings to improve knowledge and skills and to maintain competency and certification
    Maintains educational requirements with monthly learning module and other specialized training as directed by supervising nurse
    Attends and participates in care plan meetings for patient as directed by supervising nurse
    Participates in regularly scheduled interdisciplinary team meeting.
    Assists the nursing department and other departments when required in other duties and tasks when asked.
    Other duties as assigned by Patient Care Manager

    Qualifications:

    MINIMUM QUALIFICATIONS:
    Minimum of one year full-time experience in direct client care in an institutional setting (hospital or nursing facility); or one year full-time experience within the last five years in direct client care in an agency setting; No lapse of 24 consecutive months not providing services for compensation; inpatient hospice experience preferred but not required
    High school graduation or GED
    Possession of a valid Texas Certified Nurse Assistant certification
    Successfully completed training as described in 418.76
    Ability to read, write and effectively communicate in English
    Computer knowledge preferred
    Have a clear rating on the Texas Department of Human Services nurse aide registry with no finding against the aide relating to client abuse or neglect or misappropriation of client property
    Ability to work independently with little and to take direction for caseload assigned by and performed under the supervision of a RN.
    Ability to acquire new knowledge to perform additional or more difficult tasks
    Ability to maintain a positive, stable disposition in performing, working in association with staff, patients and families
    Customer centered, hospitality focused attitude while performing essential duties
    Ability to maintain a high level of performance in an environment of frequent demands and changes
    Knowledge in performing personal care services, ability to meet new and/or stressful situations with the appropriate responses, ability to work effectively as part of a healthcare team, to share information freely, to use and seek supervision and consultation verbally and in writing, and to be able to follow both verbal/ written instructions
    Ability to perform Essential Duties as outlined below
    Genuine caring for and interest in the care of mature adults, handicapped people and hospice Patients
    Ability to perform work tasks within the Physical Demand Requirements as outlined below
    Ability to comply with the Dying Persons of Rights and the Employee Responsibilities
    Ability to comply with organizational and departmental safety policies and procedures
