    Job Description

    This position is grant funded. Therefore, it is solely dependent of availability of grant funds.

    If assigned to Nurse-Family Partnership:
    Creates awareness and motivation on risk reduction to all maternal-child health hazards and promotes healthier lifestyles to general and at-risk populations. Develops and manages health education programming that reduces parental stressors in order to build improved quality of life at the individual, group and population settings as part of the overall strategy of the Parental Crisis Support Project.
    Manages coordination of basic needs support for health education recipients in order to improve program effectiveness.

    ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
    The intent of this job description is to provide a representative summary of the major duties and responsibilities performed by employees in this job. Employees may be requested to perform job-related tasks other than those specifically presented in this description.
    • Supervises, plans, schedules, and assigns work to subordinates; instructs and trains in correct methods and procedures; reviews and evaluates employee's performance.
    • Monitors and evaluates progress of assigned programs.
    • Develops and revises educational materials and curriculum on a yearly basis.
    • Conducts follow ups including but not limited to, educational, intervention and case management on a case by case basis.
    • Develops need assessments, goals, objectives, training methods and promotional activities of specific health education programs.
    • Designs and develops educational materials and conducts training sessions and workshops.
    • Performs one on one education with program participants and ensures they receive accurate and up to date information.
    • Prepares health education information for the general public and community action groups to provide appropriate risk communication.
    • Conducts presentations on health education to neighborhood and community groups.
    • Recruits, trains, organizes and utilizes volunteers in health promotion projects.
    • Follows up on referrals with phone calls, post cards, and or home visits to ensure that the participant will adhere to the regimen recommended by the provider and to motivate individuals to participate in community activities.
    • Coordinates in-service training to community volunteers on health promotion and ensures all volunteers receive proper training in order to perform work.
    • Performs a time schedule for phases of accomplishments.
    • Conducts health fairs and training sessions and assists other health department staff in promoting disease prevention activities; also coordinates with other public health programs and activities both for the department as well as other agencies.
    • Prepares written reports of current activities, accomplishments, and plans up coming activities.
    • Assists in developing audio-visual aids; operates and maintains audio-visual equipment for activities.
    • Performs other related work as assigned in a private and confidential manner, especially in health education.
    • Disseminates information to the media on risk reduction, parental mental health, family health and maternal-child health.
    • Performs other duties as assigned.

    If assigned to Nurse-Family Partnership:
    • Coordinates with community agencies to support access to mental health services for health education participants including facilitating group sessions and support groups.
    • Maintains contact with community agencies providing socio-economic and health-based services/programming including pregnancy centers, food banks, medical and human service providers, etc.
    Minimum Qualifications

    Required Education and Experience
    • Bachelor's degree from an accredited* college or university in Education, Social Work, Medicine, or Public Health.
    • At least two (2) years of experience in health education and health wellness promotions.
    OR
    • 60 college hours from an accredited* college or university.
    • At least three (3) years of experience in health education and health wellness promotions. Phlebotomy training is preferred.
    OR
    • High School Diploma or G.E.D.
    • At least five (5) years of experience in health education and health wellness promotions. Phlebotomy training is preferred.
    Proof of TB Test/Screening will be required to be presented prior to employment.

    *Council of Higher Education Accreditation (C.H.E.A.)
