    Position: Healthcare Coordinator

    Location: Kaneohe, HI

    Pay: $87,000/yr

    Experience:

    • One (1) year of related work experience in a clinical setting and/or in a managed care environment
    • Previous behavioral health case management, health plan experience, or other related work experience

    Education: Associate's Degree in Health Care Administration, Nursing, Long Term Services and Supports, Public Health, Behavioral Health, or related field

    Type: Full-time; Direct Hire

    Schedule: Monday to Friday, 8am to 5pm

    Kahu Malama Nurses is seeking a Healthcare Coordinator to join our growing and dynamic team

    • Conduct face-to-face or telephonic TOC assessments on members before they are discharged
    • Interact with members, family, physician(s), and other providers utilizing clinical and social knowledge to determine the member's status and capacity
    • Assist members with scheduling the PCP and/or Specialist post-discharge appointments, transportation, and DME needs
    • Educate members on preventative health care measures and medication
    • Upon member discharge, ensure PCP/Specialist has current member information including the discharge instructions, summary, medication list, etc.
    • Assist members with outreach to Community Health Centers, primary care providers and/or Specialists as requested
    • Maintain accurate documentation and records of TOC activities in the information system (Guiding Care/G8)
    • Ensure compliance with all state and federal regulations
    • Participate in meetings, trainings, in-services, and Inpatient and Behavioral Health Rounds
    • Other duties as assigned

    Position Requirements:

    • Licensed Clinician to include LPN, RN, LSW, or LCSW in the State of Hawaii
    • Possession of a valid driver's license with access to a reliable automobile
    • TB Clearance and First Aid and CPR Certification
    • Experience in clinical assessments, discharge planning, and utilization management
    • Intermediate computer skills and experience with Microsoft Office products
    • Must be organized and detail-oriented with excellent problem-solving abilities
    • Strong interpersonal skills while effectively working within a team environment
    • Ability to work in a high-stress environment, with sporadic fluctuations in workload
    • Excellent written and verbal communication and presentation skills
