- Facilitate access to appropriate entitlements including Medicaid, Medicare and other services provided by the Ohio Department of Job and Family Services
- Collaborates as appropriate and coordinates care with social service providers, medical care providers, family members, landlords, and other Nord Center staff associated with consumer's treatment plan.
- Services may include, but are not limited to skills training, recovery, advocacy and linkage to supportive services to meet medical, and entitlement needs; Ensure that follow up care is provided in an appropriate and timely manner according to medical necessity.
- Provides education, support to families, individuals, community and consultative services to consumers, groups and other service providers in a context that assures clients' rights to confidentiality.
- Develops and provides solution focused interventions and emotional and behavioral management from evidence-based treatments.
- Assists clients with linking to additional resources, developing coping strategies, establishing a support network, and enhancing personal relationships.
- Provide/Coordinate crisis intervention as needed with individuals, families, and groups, such as safety planning and monitoring
- Tracks and maintains current knowledge of treatment and shares with clinical teams of assigned clients' admissions and discharges to facilities such as jails, hospitals, and other residential or congregate settings.
- Participates actively in the Quality and Utilization Management functions, including aspects of care, peer review, client satisfaction, utilization review, standards of care, record review, and outcome indicators.
- Able to demonstrate high standards of organization in order to complete all functions of the TBS Specialist position.
- Provides education, support to families, individuals, community and consultative services to clients, groups and other service providers in a context that assures clients' rights to confidentiality.
- Ensures clients and families know and understand who serves on their interdisciplinary team, each member of the teams' role, as well as their level of involvement with their care.
- Completes all necessary documentation to service delivery. Clinical documentation is completed within the 24 hour guideline and reflects services rendered as well as medical necessity.
- Conducts the National Outcomes Measures Survey (NOMS) survey tool on all clients in order to obtain baseline, six month progress updates. Enters information into the EMR and SPARS.
- Provides written communication for necessary services inside and outside the Center.
- Completes Individual Service Plans and Outcomes within the time-guidelines of their due dates (initial, 6 months and annual).
- Maintains required productivity expectation.
- Attends required staff meetings.
- Provides written communication for necessary services inside and outside the Agency.
- Ability to adjust to a rapidly changing caseload, due to the potential short-term needs of clients. Reacts to change and stress productively. Gets along with others and works as a team. Maintains proper/professional boundaries with others.
- Performs related administrative duties and special projects.

Care Coordinator: Child and Adolescent Counseling - Lorain, United States - The Nord Center
Description
ESSENTIAL JOB FUNCTIONSIn order to perform the functions of this job, the employee must possess the following: a working knowledge of Nord Center Policy and Procedures, including sensitivity and adherence to clients' rights, confidentiality, and health and safety issues. Must exhibit sensitivity to different cultures.
WORKING CONDITIONS
The work is provided in a variety of venues and locations including in the office, in client homes, out in various community locations and businesses. Because of the variety of environments and physical conditions that will be encountered, this individual must be able to successfully interact and navigate in all types of facilities and weather conditions. This includes being able to walk, climb stairs, navigate changing surface conditions including wet, slippery, and icy grounds, and function in the proximity of electrical appliances, open flames and in areas with poor air quality including potentially smoke-filled rooms. This individual may periodically be required to lift and/or carry up to 10 pounds. Must be able to physically interact with client and others in emergency situations to prevent harm to client and others. The individual in this position must be able to communicate clearly both verbally and in writing. Due to the extensive computer work this individual must be able to sit for extended periods of time and operate required equipment.
REQUIREMENTS/QUALIFICATIONS
Bachelor's Degree in Psychology, Social Work, Sociology or related field and prior experience required. An Associate's Degree (or at least junior standing in a Bachelor's level program) in Human Services, Psychology, Social Work or related field and at least 3 years of full-time employment experience working directly with individuals with severe and persistent mental illness will be considered in lieu of the Bachelor's Degree. Experience in the field of mental health is preferred. Basic computer skills required, including familiarity with Windows and Microsoft Office. The necessity for travel in this job requires that the employee possess a valid Ohio driver's license, a clean driving record, have proof of adequate personal automobile insurance, and use of a personal vehicle. Knowledge of Dialectical Behavior Therapy is a plus. Bilingual (English/Spanish speaking) a plus. Experience with EHR (Electronic Health Records) a plus.
