    Description

    Position Details:

    The RN Supervisor will be responsible for overseeing the BHI department. They will collaborate with other leaders to develop, monitor, and evaluate improvement action plans aimed at enhancing the quality of behavioral health services. They will conduct weekly educational sessions and operational meetings with team members to ensure they stay informed, engaged, and aligned with organizational goals. They will ensure the timely submission of reports to Clinical Directors, providing vital insights into program performance. They will oversee the training process for team members, ensuring they receive comprehensive and ongoing training to perform their roles effectively. They will regularly audit the documentation of the Behavioral Health Care Manager daily to ensure quality and compliance with established standards. Audit the registry to ensure completion and compliance of the documentation. They will serve as a knowledgeable resource for team members, offering guidance and support on clinical matters as needed. They will supervise the quality production of behavioral health services, maintaining high standards of care and compliance. They will facilitate and ensure the monthly meetings with the Interdisciplinary Team (IDT) to review cases, and maintain program compliance, integrity, and quality.

    Responsibilities:

    • Provides assessment and care management services, including: The administration of validated rating scales
    • Initiates behavioral health care planning concerning behavioral or psychiatric health problems
    • Revises / modifies care plans for patients not progressing or whose status changes
    • Brief psychosocial interventions if needed
    • Ongoing collaboration with the billing practitioner
    • Maintenance of the registry/tracking sheet C
    • Consultation with the psychiatric consultant
    • Has a continuous relationship with the patient.
    • Collaborative, integrated relationship with the rest of the care team
    • Conducts an interdisciplinary care plan meeting to go over the patient's beneficiary to the program

    Qualifications:

    • Must be an RN
    • Long-term care/Nursing home experience
    • Managerial skills
    • Computer literate
