SASH Embedded Mental Health Clinician (SASH Clinician)
Are you ready to make a meaningful impact on individuals within affordable housing communities? We're seeking a dynamic and dedicated SASH Clinician to provide whole-person care coordination and support at our SASH affordable housing site(s).

NKHS is a 501(c)(3) private not-for-profit organization operating with the purpose of promoting high quality, comprehensive community mental health programs in Vermont's beautiful Northeast Kingdom. Our mission is to empower individuals, families, and communities by promoting hope, healing, and support. NKHS is a Designated Agency contracted with the State of Vermont to provide the highest quality of service in the areas of adults with intellectual/developmental disabilities, children and youth with serious emotional disabilities, and adults with mental health and substance use challenges. We serve the populations of Caledonia, Essex, and Orleans counties through a wide variety of targeted programs for individuals of all ages. NKHS has over 450 employees providing case management, community and home supports, residential care, psychiatry, medication management, therapy, vocational supports, school based counseling, emergency care, and respite services. Essential Duties and Functions
Direct Services:
Provide diverse therapeutic support, including individual, couples, and family therapy through in-person, phone, and telehealth sessions, conduct assessments and screenings to connect individuals to appropriate resources, lead group programs on various topics, and offer additional services such as conflict mediation, Advance Directives assistance, DNR/COLSTs, and housing support.
Care Coordination:
Coordinate care through regular meetings with embedded SASH staff, monthly team meetings, and outreach to primary care, psychiatric, and other community partners.
Consultation and Training:
Provide training for SASH Coordinators and Wellness Nurses, covering participant challenges (mental illness, substance misuse, etc.), develop and deliver workshops for agency or SASH staff and other populations, and address identified needs from staff, residents, or direct observation.
Administrative:
Utilize SASH data management system(s) for outcome data.
