- Independently assess, coordinates and implements a timely, safe patient discharge plan to the appropriate level of care. Participates in interdisciplinary approach to patient care management.
- Works with complex patient populations who have high acuity chronic illnesses.
- Develops treatment plans, provides patient and family education and serves as a professional resource.
- Provides emotional support and psychosocial counseling to patients.
- Performs other duties as required.
- Masters in Social Work from an accredited Institution.
- At least 2 years of relevant social work experience preferred.
- Experience in an Acute Care setting or post-acute care setting desired but not required.
- Positions in this classification require LMSW license in the state of Georgia.
- Must have working knowledge of software applications. ACM, CCM preferred.
- This position may be eligible for shift differentials.

PHYSICAL REQUIREMENTS (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks
