- Active RN license in the state of MN, in good standing.
- At least 12 months of nursing experience within the last 5 years; current clinical knowledge is essential if not recently active.
- One year of hospice care experience preferred.
- Reliable transportation, valid driver's license, and proof of auto insurance.
- CPR Certification.
- Conduct initial and ongoing assessments of patients and families to determine hospice needs.
- Develop and implement a comprehensive plan of care in collaboration with the Interdisciplinary Group (IDG) and the attending physician.
- Coordinate care among IDG members and external healthcare providers to ensure continuous assessment and effective service delivery.
- Administer medications and treatments as prescribed, adhering to nursing standards of practice.
- Educate patients and families on pain management and treatment responses.
- Supervise the services provided by LPNs and Hospice Aides assigned to patients.
- Maintain accurate and timely documentation of services provided.
- Regularly attend IDG meetings to ensure cohesive care.

Position Details
Hours: Full-time | Days | General working hours: Monday - Friday, 8:00 AM - 5:00 PM | Participate in an On-Call rotation.
Territory: You will serve a designated area within the South West section of the Twin Cities Metro, providing in-home hospice care to our patients.
