POSITIONSUMMARY: Primary functions are assessing, planning,delivering, coordinating, teaching and providing patient diseasemanagement (anticoagulation, hepatitis C, lipid, smoking cessation,vaccinations, etc.) care/treatment and department services. Onoccasions assumes charge responsibility. Serves as preceptorfor new hires, students and resource person for otherstaff.
RequirementsMINIMUMQUALIFICATIONS:
EDUCATION: Graduate ofan ACPE accredited School of Pharmacy. Bachelors of Science inPharmacy or Doctor of Pharmacy required.
CERTIFICATION/LICENSES:Licensed Registered Pharmacist (RPh) in the State ofNew Mexico.
Licensed Pharmacist Clinician (PhC)by the New Mexico Board of Pharmacy
Registeredas a Pharmacist Clinician with the Board of Medicine to exerciseprescriptive authority in accordance with current/active guidelinesor protocols as defined by the Pharmacist PrescriptiveAuthority Act, Section 61-11B-1 through 61-11B-3 NMSAand NMAC-Rn
Certification in BasicLife Support for Healthcare Providers (BLS)
Advanced training/certification in anticoagulation and/ordisease management
SKILLS:Gathering and analyzing physiological, psycho-socialand socio-economic data; accurately evaluating patient condition inperson or via phone; recognizing changes in patient status andordering appropriate diagnostic tests and therapies; educating,listening and communicating with patients and families, physiciansand other staff.
Ability to read, write,and speak English fluently required; fluency in Spanishpreferred. Strong verbal, written and interpersonalcommunication skills. Ability to work collaboratively withothers. Clinical pharmacy skills including physicalassessment, planning, delivering and evaluation of carerequired. Ability to work at a rapid pace under stressfulconditions.
EXPERIENCE:Direct patient care experience as PharmacistClinician with prescriptive authority. Experience current indisease management
NATURE OFSUPERVISION:
-Responsible to:Anticoagulation Management Services Coordinator andPharmacy Director
ENVIRONMENT:May be exposed to physical injury, toxic substances,infectious diseases and bloodborne pathogens. Requiresknowledge of safety standards to limit exposure to infectiousdiseases.
