    Job Description

    The Medical Records Specialist is responsible for maintaining all medical records and entering client data into the electronic medical records software. The Medical Records Specialist must stay compliant with HIPAA, respond to record requests, and perform quantitative review of records.

    Specific duties include:

    Creating, activating, maintaining and deactivating all medical records.

    Maintaining client data in Access database and filing all medical records as needed.

    Tracking client admissions and discharges.

    Data entry into LME/MCO system per MOA requirements.

    Tracking and completing requests for medical records to and from the agency and advising appropriate staff of their request for medical record data. Ensure HIPAA compliance (APSM 95-2, 455-2, 45-1) regarding release of personal health information to outside agencies before requests are completed.

    Identify and communicate patterns of

    Assist Quality Improvement Director, Coordinator, or Specialist with tracking and trending all client incidents.

    Participation in quality improvement program including health and safety.

    Scheduling evaluations for clients (i.e. psychological, sleep studies, neuropsychology, etc.)

    Tracking and processing prescription requests including communication with the Nurse Practitioner and/or Psychiatrist.

    Perform other related duties and special projects as assigned.

    Must display good communication and feedback within a team dynamic.

    Must display professionalism in all areas of work including client and co-worker relationships.

    Must be proficient in Word, Excel, and Adobe.

    Must have a medical records certification or obtain such within employment probationary period.
