- Current, unencumbered Certified Nursing Assistant (CNA) license in the State of Virginia.
- CPR certification (or must obtain within the first 60 days of employment).
- Able to speak, read, and write English and shall meet one of the following qualifications before providing services to the hospice program patients:
- Have certification asa certified nursing assistant issued by the Virginia Board of Nursing;
- Be successfully enrolled in a nursing education hospice program preparing for registered nurse or practical nurse licensure and have currently completed at least one nursing course that includes clinical experience involving patient care;
- Completion of a Food Sanitation course after hire.
- Assist patients with activities of daily living; ambulation and prescribed exercise; and other special duties with appropriate training and demonstrated competency.
- Assist with mobility and positioning, including the use of assistive devices.
- Monitor and record vital signs as indicated in the plan of care.
- Provide emotional support and companionship to patients and families.
- Maintain a clean and safe environment for patients, visitors, and staff.
- Complies with current infection control policies and protocols.
- Record and report changes in the patient's physical condition, behavior, or appearance to healthcare professionals.
- Document services and observations in the medical record.
- Perform any other duties that an CNA is qualified to do by additional training and demonstrated competency within state guidelines.
- Assist with meal/snack preparation; serve food in accordance with current federal, state, and local standards and guidelines and our established policies and procedures.
- Demonstrate infection control and sanitation knowledge concerning food temperatures, hand hygiene, gloves use, and dating food items.
- Participate in quality improvement initiatives and continuing education to maintain up-to-date knowledge and skills.
- Organizes work schedule and utilizes time management to complete assignments.
- Maintains accurate and complete medical record documentation.
- Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
- Communicates to the supervisor if unable to meet a patient's need or perform a procedure.
- Immediately report any accident, incident, lost articles, or unusual occurrence to the Clinical Coordinator or Hospice Director.
- Adheres to the code of ethical behavior and models our core values of respect, passion, and professionalism at all times
- Maintains skills and knowledge, keeping all required certifications current.
- Participates in quality improvement initiatives and continuing education to maintain up-to-date knowledge and skills.
- Organizes work schedule and utilizes time management to attend all required meetings and team conferences.
- Adheres to the HHHR code of ethical behavior and models our core values of respect, passion, and professionalism at all times.
- Performs all other duties as assigned.
- Physical Requirements:
- Requires considerable physical effort most of the day, including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes, and maximal assistance in lifting and/or transferring of a 100-pound patient.
- Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position
- Must provide evidence of annual TB test and other state-required tests or examinations.
- Mental Requirements:
- Must be able to work independently, make judgments based on assessments and data available, and act accordingly.
- Must be flexible, innovative, and possess good interpersonal skills.
- Must be able to cope with mental and emotional stress and demonstrate emotional stability.
- Working Conditions:
- Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions, and hazardous materials.
- Reports directly to the Hospice IPU Clinical Coordinator
- Works cooperatively with all members of the Interdisciplinary Team

I. QUALIFICATIONS
