    Job Description

    The Geriatrics Assessment Program at Stamford Health is seeking a 1-2 BC/BE fellowship trained geriatric medicine physician to join an established group of 4 physician geriatricians and 2 nurse practitioners. This position reports to the Director of Geriatric Medicine. Our outpatient practice consists primarily of providing initial and follow-up consultation for patients who are referred by other physicians, or are self-referred for cognitive evaluation, geriatric syndromes, and multi-morbidity. We also provide primary care for some of the oldest patients in our community.

    The geriatric medicine team works closely with hospitalists and Internal Medicine and Family Medicine residents, emphasizing inter-disciplinary collaboration and family support. Stamford Health is highly invested in geriatrics care, with complementary programs being developed in nursing, emergency medicine and palliative care.
